Terms and Conditions: In consideration of my employment, I agree to conform to the rules and regulations of Ottawa Regional Hospital. I understand that my employment can be terminated at any time for any reason, at the option of either the facility or myself. I understand that no one has any authority to enter into any agreement for employment for any specified period of time or to make any agreement contrary to the foregoing, except for a written employment agreement signed and notarized by the President of Ottawa Regional Hospital. I hereby affirm that the information provided on this application (and accompanying resume, if any) is true and complete. I understand that any false or misleading representations or omissions may disqualify me from further consideration for employment and may result in discharge even if discovered at a later date. I understand that Ottawa Regional Hospital will conduct a criminal background check either prior to or during my first 90 days of employment. I understand that pursuant to the Illinois Health Care Workers Background Check Act, if this check reveals a conviction for one of specified prohibited crimes, I will be terminated from any direct patient care position. Ottawa Regional Hospital will provide a pre-employment physical as part of the conditional offer of employment which includes a drug screen. I understand that if the results of any part of the physical fail to meet the Hospital’s standards, the conditional offer of employment may be withdrawn.I have applied to Ottawa Regional Hospital for employment. I request and authorize you to provide any information you may have regarding me and/or my employment with you and I do unconditionally release your organization and all individuals associated with it from all liability for any damages which might result from furnishing any information. I understand that if I am a credentialed medical professional, my credentials will be verified with the appropriate licensing and disciplining authorities to determine if any adverse action has been taken against me. This verification process shall include LEIE of the Office of the Inspector General (U.S. Department of Health and Human Services), General Services Administration Debarment List and the National Practitioners Data Bank, where appropriate. Consistent with OIG compliance guidelines, if a background check reveals that I have been sanctioned/excluded from any federal or state healthcare program, I will be terminated.
